    Your best engineer isn’t quiet because they’re focused.

    There is a moment I have learned to listen for when an engineer finally talks. It is not the workload, the on-call rotation, or the reorg. It is the sentence underneath all of those: I can debug anything except whatever this is. One staff engineer put it exactly that way in therapy built for software engineers, then added that he had opened a doc three separate times to write out what was wrong with him, structured it like a postmortem, and abandoned it because he could not reproduce the bug. He is a composite of several clients, but the postmortem doc is a detail I have seen in the room more than once.

    Engineering culture rewards a specific move: take the ambiguous thing, make it legible, close the ticket. That move fails against anxiety, burnout and depression, which do not reduce cleanly and do not close. So the strongest engineers do what strong engineers do with a flaky test. They quarantine it. They route around their own distress for quarters at a time, shipping the whole way, and the shipping is exactly why nobody asks if they are okay. Good clinical work starts from that culture instead of fighting it, treating the client like someone who reasons carefully about systems and is currently trapped inside one that lacks observability.

    The concealment is not imaginary caution, and it runs all the way up the org chart. In a CEREVITY clinical review of 318 founders and venture-backed operators seen between January 2025 and July 2026, 64 percent were actively concealing mental health strain from investors or boards, and 58 percent named professional or funding consequences as the primary reason. The median delay from noticing the strain to a first session was 21 months. If the founder is hiding it from the board, and the VP is hiding it from the founder, the engineer hiding it from the VP is not being irrational. The whole ladder is running the same protocol.

    Here is what it looks like below the surface, in the language of the work itself. Code review comments get shorter and meaner, then stop. The engineer who used to argue in design docs starts approving things silently, because arguing requires caring and caring requires fuel. Side projects die first, they were the canary. Sleep breaks in a particular way, not trouble falling asleep but the 3 a.m. wake with the mind already compiling. And there is a distinctive shame loop in this population: the belief that a mind good at logic should be able to out-think a mood, so every failed attempt to reason with the depression becomes further evidence of personal defect. It is a bad theory of mind, and dismantling it is often the first real work of therapy.

    For the people running teams the stakes compound, and the failure mode changes shape. Tech leaders carry the engineer’s cognitive load plus the emotional labor of absorbing everyone else’s, usually with no one above them they can be honest with. The composite CTO in my notes did not miss a launch. He describes making two mediocre architecture calls in a row, knowing they were mediocre while making them, and being too depleted to generate the third option he could feel existed. That is what burnout costs a technical organization. Not absence. Judgment.

    What works clinically with engineers is refreshingly concrete. They want a model of what is happening to them, and there are good ones. They want instrumentation, and symptom tracking gives them real telemetry. They want the work to respect their intelligence without letting them intellectualize the feeling out of the room, which is the actual craft of treating this population. Progress tends to be visible within weeks once the quarantine ends, because the same persistence that maintained the concealment gets pointed at recovery.
